Schuylkill Co., PA – 1 Killed, 2 Injured in Three-Vehicle Crash on I-81 SB Near MM 135.5

Schuylkill County, PA (November 15, 2025) – A deadly three-vehicle crash occurred on Friday, November 14, near mile marker 135.5 on Interstate 81 South, according to Pennsylvania State Police in Frackville. The collision happened around 5:49 p.m. and involved a 2024 Honda Civic, a 2024 International tractor-trailer, and a 2016 Audi Q5.

Investigators reported that the chain reaction began when the Honda Civic, driven by Rafael Ceballos of Hazleton, re-entered the right lane after passing a vehicle stopped on the west shoulder. As he merged back into traffic, the front right side of his Honda struck the rear left side of an International tractor-trailer driven by Charvel Prophete of Columbus, Ohio.

After the first impact, the Honda collided with a 2016 Audi Q5 driven by Danyel Miller of Shenandoah, who was travelling in the left southbound lane. The Audi left the roadway, went down an embankment in the median, and rolled several times before stopping in the left lane of I-81 North. Miller sustained fatal injuries and was pronounced deceased at the scene by the Schuylkill County Coroner’s Office.

The Honda spun multiple times and came to rest in the right lane of I-81 South, facing north. The tractor-trailer was able to stop safely on the west shoulder.

Ceballos and Prophete suffered minor injuries. McAdoo Fire Co. Ambulance transported Ceballos to Lehigh Valley Hospital–Hazleton, while Shenandoah Community Ambulance took Prophete to Lehigh Valley Hospital–Schuylkill East.

All vehicles involved sustained disabling damage, except for the tractor-trailer, which had minor damage on the rear side.
